wear blue: run to remember

2025 Gold Star and Survivor Endurance Program MCM Application

Application Form

The mission of the Gold Star and Survivor Endurance Program is to empower our surviving family members on their journeys of healing and remembrance.

Throughout the program we will have regular conference calls, email discussions and other forms of communication. It is our way to connect with each of our participants and understand your training needs, discuss possible injuries and to motivate and best support each other through the process.

For us to apply for funding for the upcoming year, we need to provide our future funders with the demographics of our participants. This funding allows us to support and expand our programming. Please fill out a few optional questions below.
